- Child Registration Certificate (B-Form)The Child Registration Certificate (CRC), also called the B-Form, registers a child in NADRA's records. Both parents apply in person at a NADRA counter, such as at a Punjab e-Khidmat Markaz, with their CNICs and the child's birth certificate.
