پیدائشی سرٹیفکیٹ (پنجاب)
In Punjab you can register a birth and get a computerized birth certificate through e-Khidmat Markaz; the certificate is issued by the relevant field office of the district where the case is registered. Cases within 60 days of birth need Form A, the hospital birth slip, vaccination card and family CNICs; cases after 7 years must go through court.
پنجاب میں آپ ای خدمت مرکز کے ذریعے پیدائش کا اندراج کروا کر کمپیوٹرائزڈ پیدائشی سرٹیفکیٹ حاصل کر سکتے ہیں؛ سرٹیفکیٹ اسی ضلع کا متعلقہ فیلڈ آفس جاری کرتا ہے جہاں کیس درج ہو۔ پیدائش کے 60 دن کے اندر درخواست کے لیے فارم اے، ہسپتال کی برتھ سلپ، حفاظتی ٹیکوں کا کارڈ اور خاندان کے شناختی کارڈ درکار ہیں؛ 7 سال کے بعد کے کیس عدالت کے ذریعے ہوں گے۔

- 2Get the photocopies attested by an officer of grade 17 or above.فوٹو کاپیوں کی تصدیق گریڈ 17 یا اس سے اوپر کے افسر سے کروائیں۔
- 3Visit your nearest e-Khidmat Markaz (Monday to Saturday, 9:00 am to 5:00 pm) with the originals and copies.اصل کاغذات اور کاپیاں لے کر اپنے قریبی ای خدمت مرکز جائیں (پیر سے ہفتہ، صبح 9 بجے سے شام 5 بجے تک)۔
- 4Pay the Rs. 200 fee in cash and keep your tracking receipt.200 روپے فیس نقد ادا کریں اور ٹریکنگ رسید سنبھال کر رکھیں۔
- 5Collect the original certificate after 7 working days.7 ورکنگ دن بعد اصل سرٹیفکیٹ وصول کریں۔

- • Payment is in cash, in Pakistan Rupees.
- • Cases are processed from the applicant's relevant district where the case is registered; Cantonment Board cases are processed by the relevant Cantonment Board.
- • e-Khidmat Markaz is open Monday to Saturday, 09:00 am to 05:00 pm (no lunch break; closed 12:30 pm to 02:30 pm on Friday). Lahore - ASTP and Lahore - Town Hall also open in the evening, 06:00 pm to 12:00 am, Monday to Saturday.
